Appointments and Services for Veterans
ADF DVA Transition Medical Appointment
This is typically the first appointment with a GP, often arranged just prior to discharge from the ADF. Your doctor will review your medical history, assess your immediate needs, and discuss care programs or support services you may benefit from.
CVC (Coordinated Veterans Care) Program
The CVC program is designed for veterans with complex health needs. Led by our nursing team, it includes:
- Monthly phone check-ins with your nurse
- A three-monthly review with your nurse and GP
- A team-based approach to ensure continuity and quality of care
You may be eligible if you have a DVA Gold Card or a White Card with accepted mental health conditions. This program is fully funded by DVA if all scheduled appointments are attended.
ADF Discharge Health Assessment
Available to veterans who have discharged from July 2019 onward, this annual comprehensive health assessment is fully DVA-funded for up to five years post-discharge.
- 50-minute nurse consultation (history and observations)
- 20-minute review with your GP
Veteran’s Health Check
This is a one-off comprehensive assessment available to all veterans. It includes:
- 50-minute consultation with a nurse
- 20-minute appointment with your GP
It’s designed to provide a full health snapshot and set up ongoing care.
DVA Reports
We understand the paperwork can be daunting. Our lead GPs, Dr David Yates and Dr Renee Robarts, are experienced with DVA reports and are happy to complete them. Other doctors may also assist under guidance to ensure timely processing.
Once you’re a patient, please email your documentation to veterans@tsaj.com.au and our Admin Manager will contact you to arrange a suitable appointment.

Your DVA White Card Conditions
To ensure we can bill DVA correctly and provide the care you’re entitled to, we’ll need a copy of your DVA White Card and the list of approved conditions.
Please visit the DVA website (not the app), download your White Card and condition list as a PDF, and email it to us at veterans@tsaj.com.au so we can upload it to your medical record.
